Community Solutions
Strong partnerships between city departments, CBOs, and the UCSF and ZSFG health system enable more coordinated care between programs. This collaborative model ensures that residents can move seamlessly to the type of support that best fits their needs. In the Bayview, The San Francisco Pregnancy Family Village (SFPFV) gathers participating organizations together in one place through monthly events to improve access, experience and outcomes for pregnant residents, especially Black families. In addition, the program brings participating organizations together outside of events for "anti-racism" learning and practice to benefit the community members they serve.
Person-Centered Care
These programs start with what the patient needs and shape services around their lived experiences. Hive Clinic provides care that is stigma-free and identity-affirming, ensuring that people living with or affected by HIV feel safe, respected, and empowered in their prenatal care. Team Lily adapts care to fit patients’ lives, offering flexible appointments, outreach outside the clinic, integrated mental health and addiction support. To build trust, Team Lily sends navigators and social workers to meet pregnant patients in shelters, encampments, and homes.
Advocacy
Advocacy is built into how these programs deliver care, shift systems, and elevate community voice. Across all three programs, advocacy is both practical and structural. This looks like:
- Supporting individual patients in real time (e.g., open access clinics, navigating benefits, housing, legal issues, providing trauma-informed care.)
- Pushing for broader change (e.g., anti-racist care models, stigma-free health systems, more integrated social support and wrap-around services, including offering care inside the SF County jail)
Our Programs
SFPFV is a group of San Francisco city agencies, healthcare organizations and community-based organizations committed to improving care access, experience and outcomes for Black pregnant families in San Francisco. Together they create community events to provide a reliable one-stop-shop of offerings, built around the community’s needs.
Team Lily, a clinic at Zuckerberg San Francisco General Hospital, was designed to make it easier for patients affected by mental illness, substance use disorders, homelessness, and intimate partner violence to access prenatal and postpartum care. The multidisciplinary care team provides wraparound services in and outside traditional clinic settings.
HIVE Clinic is a clinic dedicated to providing care to people living with HIV in and around San Francisco and is based at Zuckerberg San Francisco General Hospital. Our mission is to advance reproductive and sexual wellness for individuals, families and communities affected by HIV in San Francisco. We envision a world where people affected by HIV have safe pregnancies, reproductive autonomy, access to state-of-the-art health care.
